A pilot of a hybrid clinic for COPD patients

The Sheba BEYOND Hospital, under the guidance of Dr. Galia Barkai, in partnership with Dr. Erik Baltaxe, a distinguished physician from the Sheba Lung Institute, and AstraZeneca, is pioneering a holistic and innovative pilot programme. It offers a hybrid approach to treatment and follow-up care designed for COPD patients who have been discharged after an exacerbation of their condition.

What is COPD?

COPD is the world's most widespread chronic lung condition, and occurs primarily from inhaling harmful substances like smoke, which irritate and damage lung tissue. 
Early signs of COPD involve inflammation of the peripheral airways, leading to the destruction of lung tissue (emphysema), respiratory issues, and changes in both inflammation and pulmonary circulation. Frequent acute attacks significantly increase the risk of worsening the condition and hospitalisation.
 

What does the program include?

The program uses a hybrid approach, blending evidence-based treatments with tools like saturation meters for checking oxygen levels, smartwatches, and home spirometers for lung tests.
It aims to improve patients’ quality of life and boost their self-confidence, which should also reduce the pressure on hospital admissions.
Previous studies have found that home-based and technology-assisted treatments not only make patients happier but also lead to better health outcomes.
Additionally, the program offers pilot participants tools to enhance therapeutic sequences and ensure a smooth transition from hospital to home care within the community. This is facilitated through a secure online medical services platform, aiming to reduce hospital stays, prevent readmissions, and avoid unnecessary emergency room visits.
The program was created at the ARC Innovation Center using a hybrid care model (MICCA), which enables care and follow-up at home tailored to the patient's needs through technology and a readily available multidisciplinary team.
